Social Work Services

Registered Clinicians with a Master’s of Social Work offer counselling using various approaches that are individualized to best meet the patient’s goals and treatment plan. Therapy is available to patients enrolled at the Erie Shores Family Health Team. Therapy involves a range of services from individual counselling to attending a group education/support session. Social Worker staff work closely with patients to determine the therapy service that best addresses the health care needs.

Referrals

Patients and/or health care provider can refer to Social Work. It is important to know there is currently a waitlist for Social Work. Once a referral is made, the patients name is added to the waitlist. When a Social Worker becomes available, the patient is called to schedule an appointment to begin the assessment process.

What to expect from counseling:

Counseling is time limited and supports the patient to become independent as quickly as possible.

Counselling is multi-pronged: educational, future focused and goal directed.

Private insurance such as EAP must be accessed first before ESFHT counselling is available.

Patient Responsibility during counselling

Attend sessions on time and cancel in advance if you cannot attend.

Complete any homework or skill practice between sessions.

Understand that fully participating in counselling is hard work and takes commitment!

Group Workshops and Sessions

Provides free online, self-help, and evidence-based resources on anxiety. Access simple, step-by-step instructions on how to deal with symptoms related to specific anxiety disorders as well as general strategies.

Bounce Back

This is a free skill-building program managed by the Canadian Mental Health Association (CMHA). It is designed to help adults and youth 15 = to manage low mood, mild to moderate depression and anxiety, stress or worry. Delivered by phone with a coach and also access tools using online videos.
