ESFHT Uninsured Services

Some services are not covered by the Ontario Health System and therefore must be paid by the individual patient requesting the service. Examples of such uninsured services may include:

  • Completion of forms for physicals for: schools, camp, pre-employment, healthcare employees.
  • Completion of Licensing forms for: ministry of transportation examination, driver’s medical, civil aviation medical exam report, pilot’s license validation.
  • Back to Work/School related forms
  • Other: insurance treatment, disability/maternity, compassionate care benefits, catastrophic impairment, life insurance, medical disability, application form for prospective foster/adoptive parent, citizen and immigration medical report, applicants to Canadian Armed forces, life and health insurance reports and assessment fees, wart removal, minor cosmetic procedures.

Patients are encouraged to confirm the fee schedule with the provider prior to receiving any uninsured service(s). Cash or cheque payment for uninsured services is required at the time the service is provided.

Urgent
Care Appointments

Urgent care appointments are for ESFHT patients with an unexpected but non-life threatening health problem that requires same-day or next-day treatment.
