Quality Improvement
The Erie Shores Family Health Team works closely with Health Quality Ontario to ensure health care is provided to address provincial and local priorities such as:
- Timely and efficient transitions in care. For example, when a patient is discharged from the hospital, there is timely access to a primary care provider for patient follow-up and support.
- Service excellence. This is related to the health care team involving the patient in decisions about their health care.
- Providing safe and effective care. This is exemplified through use of best practice in all areas of care, including, but not limited to, privacy, accessibility, palliative care, opioid prescriptions, patient safety, to name a few.
The above priorities are accomplished with an emphasis on improving the patient experience, ensuring value and efficiencies within the health care system, enhancing provider experience, all while promoting the overall health of the population.
To achieve ongoing quality improvement, the Erie Shores Family Health Team develops and implements an annual Quality Improvement Plan (QIP) to demonstrate it's ongoing commitment to the provincial priorities as listed above.
The Erie Shores Family Health Team Quality Improvement Plan (QIP)
The QIP is a public document that health care organizations use to make clear their commitment to quality. The QIP is updated annually and is used as a working document to identify needed improvements and to assess progress toward improvement by monitoring performance on a set of quality indicators. Throughout the year, the ESFHT reviews performance, making strategic adjustments to support success.
Front line team members, leadership, the patient advisory council, board members, patients and their families can all offer input into quality improvement activities that include mandatory indicators as well as local priority indicators.
