Pharmacist Support

The Erie Shores Family Health Team (ESFHT) has one full time Pharmacist on the team. In addition to supporting the Smoking Cessation and INR programs, the ESFHT Pharmacist also:

  • Develops medication maps for patients requiring additional support to correctly use prescribed and recommended medication
  • Provides education and support to patients recently discharged from hospital to ensure newly prescribed medications are correctly integrated into the medication regimen
  • Is an active participant during Memory Clinics to review and discuss the influence of medication on memory and general functioning 
  • Works as a resource to all members of the ESFHT to support safe medication prescribing and use.

Referrals

Patients and/or health care providers can refer to the Pharmacist.

What to expect from the Pharmacist

Once a referral is made, the pharmacist contacts the patient by telephone to complete an assessment to guide an individual treatment plan.

Patient Responsibility

Attend appointments (office or phone) on time and cancel in advance if you cannot attend.

Attend office appointments with all your medications, if requested.

Follow pharmacist recommendations.

Urgent
Care Appointments

Urgent care appointments are for ESFHT patients with an unexpected but non-life threatening health problem that requires same-day or next-day treatment.
